Child - A
Age/Year group – 4B 8 years 7 months
Male/Female - M

Type of behaviour/diagnosis (disorder): Inattentive

Comorbid diagnosis (disorder): Possible ADHA child psychiatrist from CAMS being in to observe him in school environment and now waiting on a diagnosis

Context and setting: In year 4 class of 30 pupil with teacher and one TA

Classroom
• Sitting on carpet for more than 5 mins he starts disrupting other pupils fidgets touching pupils shouting out.

Assemblies
• Sitting for too long will fidgets, talking to other pupils and constantly truing around looking at other pupils.

Playground

• Cannot take turns in game, cannot follow rules of games and does not like losing in games. Get angry and physical hurts other pupils


Describe behaviours:
• Can’t make decisions
• Get bored easily
• Disorganised
• Forgetful
• Don’t think about consequences of his actions
• Can’t wait his turn
• Interrupting you when you’re talking
• Being unkind to other pupil focus on one child
• Shouting out in lessons
• Not listen or following instructions
• Walk out of classroom without permission
• Don’t show emotion when he upset another pupil
• Blames others
• Difficulty in handling frustration
• Can physical hurt other pupils
